Metastatic Renal Cell Carcinoma to the Pancreas: A Review.

肾细胞癌转移至胰腺:综述

Abstract

The pancreas is an unusual site for tumor metastasis, accounting for only 2% to 5% of all malignancies affecting the pancreas. The more common metastases affecting the pancreas include renal cell carcinomas, melanomas, colorectal carcinomas, breast carcinomas, and sarcomas. Although pancreatic involvement by nonrenal malignancies indicates widespread systemic disease, metastatic renal cell carcinoma to the pancreas often represents an isolated event and is thus amenable to surgical resection, which is associated with long-term survival. As such, it is important to accurately diagnose pancreatic involvement by metastatic renal cell carcinoma on histology, especially given that renal cell carcinoma metastasis may manifest more than a decade after its initial presentation and diagnosis. In this review, we discuss the clinicopathologic findings of isolated renal cell carcinoma metastases of the pancreas, with special emphasis on separating metastatic renal cell carcinoma and its various differential diagnoses in the pancreas.

胰腺不是肿瘤发生转移的常见位置,胰腺恶性肿瘤中大约只有2%-5%是由其他部位转移而来。侵犯胰腺的最常见转移性肿瘤包括肾细胞癌、黑色素瘤、结直肠癌、乳腺癌、肉瘤。非肾细胞癌来源的胰腺转移性癌表现为全身性疾病,肾细胞癌转移至胰腺通常仅为一种独立的病症,可通过手术切除术治疗,这与长期生存率有重要关系。同样地,从组织学上准确地判断胰腺癌是由肾细胞癌转移而来非常关键,应特别注意的是肾细胞癌在第一次发病及诊断十多年后或许才会发生远处转移。

本篇综述中,我们讨论了肾细胞癌单发转移至胰腺的临床病理表现,并特别强调在胰腺中如何区分转移性肾细胞癌及其多种鉴别诊断。
